Requirements for 2024 Fairs
The 4-H Project Activity Plan is for project leaders to complete. Submit this form to the 4-H office prior to starting your meetings and share all the information with parents and youth members by the first or second project meeting.
The Annual Project Report form must be completed to receive credit (stripe & pin) for the program year. This requirement is for all members.
If a member is going for a Star Rank or County Award, the Personal Development form (PDR) is also needed.
Mini member Personal Development Report
If a member was a Junior or Teen Leader for a project, the Junior and Teen Leader form is required to get credit.

If applying for County Project awards, the project report form for that project (either Annual Project Report or Market Animal forms) must be completed and added to your record book. See forms below:

County Achievement Award Application
Livestock Transfer form- if transferring to Independent or FFA, this form must be submitted prior to fair. The time-line for different species is: 120 days (beef or horse), 60 days (swine, sheep or goats), or 30 days (rabbits, chickens) before fair.
